Autologous fat grafting

Autologous tissue grafting is one of my favorite aesthetic treatments, especially with autologous fat. I have been specializing in this method for years, and my clinic in Zug offers a variety of fat grafting procedures to treat facial wrinkles and rejuvenate the skin (among other things).

Over time, the skin loses tone and elasticity; it ages at every layer, which often results in loss of volume. The cheeks are particularly prone to fat loss. Some people also experience fat redistribution: they lose volume in their faces while also developing unwanted layers of fat on their necks.

The natural loss of fat in the cheeks and other areas (such as at the temples, on the forehead, around the eyes or near the nasolabial fold) can make a person look older. Surgical procedures like face lifts are not always necessary to make a person look younger and fitter; oftentimes, unwanted wrinkles and lost volume can be corrected using minimally invasive methods. A variety of options may come into question for wrinkle treatment. In the plastic surgery world, hyaluronic acid is particularly well-established as a wrinkle filler material, but the body’s own fat is a commonly used option as well. Once injected, the autologous fat cells heal into the surrounding tissue, creating a long-lasting filling effect on the wrinkle or sunken area.

The advantages of lipofilling

The main advantage of lipofilling is that the grafted fat comes directly from the patient’s own body, so it heals into the surrounding tissue much more readily than foreign substances. This makes it a particularly well-tolerated procedure; rejection reactions are practically unheard-of.

Fat also has a beneficial effect on skin quality—it makes the skin look softer and more delicate. Over time, patients find their skin feels more refined as well. Long-term, autologous fat cells make a significantly better filler.

Treatment

Lipofilling in Zug – what to expect

After a thorough individual consultation and a precise analysis at my clinic, your lipofilling (autologous fat grafting) treatment will be scheduled for a later date. Before the fat cells can be injected into the desired areas of the face, they will first need to be harvested and prepared. This is done using a variety of methods and techniques, which have developed immensely in recent years. 

I start by performing liposuction, which is an outpatient procedure; depending on the indication, it may be done using local, twilight, or general anesthesia. I remove the necessary fat cells from the body by inserting a fine cannula directly into the fatty tissue. Ideal areas for harvesting may include, for example, the neck, the stomach, or the thighs. Once the fatty tissue has been removed, it undergoes special processing, leaving only the best-quality cells for transplantation.

The second step of the procedure is to inject these fat cells directly underneath the skin, for example around the cheeks, the jawline, or the lips. We use the grafted fat to plump up those previously defined areas and give them an aesthetically pleasing contour, or to fill and smooth bothersome wrinkles, giving the face a more youthful look. The entire procedure takes around an hour, though more extensive operations may require more time.

Results

When do the results of lipofilling become visible?

Though the initial effects of lipofilling are visible immediately following the procedure, the transplanted fat needs time to heal, so it will take a few weeks to see the final results. Some of the fat will be flushed out of the body naturally, since not all the grafted cells will “take” in their new location—the grafting success rate is usually between 60 and 80 percent. After a healing period of around six weeks, we will be able to determine whether a second round of injections is necessary to achieve optimum results.

Lipofilling starts with liposuction, which offers a second advantage: besides treating wrinkles using natural materials, we can also address bothersome fat deposits—freeing areas like the neck or the stomach from unwanted fatty tissue to leave a slimmer, more proportional silhouette.

  • Is any specific follow-up care needed after treatment?

    For the first week after undergoing fat grafting, patients should take it easy and protect the treated area from direct sunlight. Depending on how much fat was harvested and from where, it may be advisable to wear special compression clothing.

  • Does the procedure entail any specific risks or potential side effects?

    Fat transplantation is considered a very gentle and low-risk method. Since it involves material taken from the patient’s own body, there is typically no risk of allergic reactions or immune responses. Some people may experience mild, temporary swelling or redness of the face; bruising and infections are rare.

    Liposuction involves making small incisions in the skin so that the cannula can be inserted into the fatty tissue. In our experience, these heal quickly and without scarring. Though the procedure carries a small residual risk of bruising, wound healing issues, infections, and anesthesia reactions, these complications are extremely rare with experienced plastic surgery experts.
